I began working for Feel good Factor in January 2020 as the SWIFt Wellbeing Coordinator for Chapeltown. Previously I worked in the community as a family support worker.

I am now the Enhance Development and Support Worker covering the Chapeltown area. The Enhance programme is a citywide service led by the Leeds Older People Forum and aims to bring together third sector organisations with local Neighbourhood Teams to help reduce patient readmission into hospital. Although the majority of referrals will be from the Chapeltown Neighbourhood Team, the Enhance programme can accept relevant referrals from other community services.

As the Enhance Development and Support Worker I will visit people in their home once discharged from the Neighbourhood Team. Initial joint visits with the Neighbourhood Team will take place to work together to assess people’s need for support, checking safety in the home, develop a person-centred plan of action, offer 1 to 1 support to resolve any concerns or issues, help to build confidence through regular contact and befriending, linking with social and community activities to help to improve health and wellbeing.
