Linking Leeds is a type of service known as social prescribing which is a non-medical, community based intervention that offers people the opportunity to improve and enhance their day to day live.

People who would like to access the service see a Wellbeing Co-ordinator who works closely with them to find out their needs, then helps them to access relevant activities, including art, gardening, fitness groups, healthy eating or other specialist advice and support services.

 

Linking Leeds replaces three social prescribing initiatives in Leeds – Connect Well, PEP and Connect for Health – bringing them into one integrated service available to all Leeds residents. It is funded by NHS Leeds Clinical Commissioning Group, led by Community Links in partnership with BARCA, Leeds Mind, Feel Good Factor, Better Leeds Communities, Leeds Irish Health and Homes, Age UK and Leeds City Council.
