My role as Care Co-ordinator involves developing and co-ordinating holistic, person-centred care plans with people living with frailty in Chapeltown, with the aim of promoting health, wellbeing and independence, and reducing isolation.

My career in the third sector began supporting young women with learning disabilities and additional needs. Since then, I have worked with people with disabilities around personalisation and direct payments, and developed a training service for people employing their own personal assistants. I have also managed a service delivering training and work skills for adults with learning disabilities and autism. With over 20 years experience of supporting people I am happy to bring my skills to this exciting role with FGF.
