I am a Social Prescribing Link Worker in the Chapeltown Primary Care Network. I have been involved in supporting people over the last 30 years in many different areas.

As a Link Worker, my role involves supporting people to manage their own long term health conditions. I feel it is imperative to work holistically with people and build up the relationship to support them to manage their health conditions. This will hopefully lead onto less social isolation and more engagement in the services in the area so they can become more independent and maintain their physical and mental well-being.
