CARE CO-ORDINATOR

Salary £23,501 per annum pro rata (£18,801 actual) / 28 hours per week (35 hours per week FT equivalent)

As an enthusiastic Care Co-ordinator, you will be involved in proactive, person-centred care planning with local people. This is a role where you can make a real difference by supporting individuals to engage within their local community and pursue their personal goals, in line with their choices and preferences. The work will be undertaken in ways which are socially inclusive with the aim of empowering people to live healthier, happier and more independent lives.

COMMUNITY BUILDER (CARERS)

Salary £22,816 per annum pro rata (£13,690 actual) / 21 per week (35 hours per week FTE equivalent).
Closing date Friday 30 October 2020 at 5.00pm / Interviews will take place on Wednesday 18 November 2020.

As a Community Builder you will work with the local community in the Harehills area of Leeds to support carers from BAME communities to be able to have a short break from caring using the Asset Based Community Development (ABCD) model. The Community Builder will focus on engaging the skills, knowledge and talent of local residents through relationship building. This will identify, engage and motivate Community Connectors who will lead positive change. The Community Builder will also be responsible for a small fund to support community activities. As an independent self-starter, you will understand how to work collaboratively and how to engage and bring together carers in the Harehills area.
