The SWIFt Service is for people aged 50+ living with frailty or having complex health issues and who are at risk of being socially isolated. The service offers one to one support to help people living in the Chapeltown area to identify their needs and to improve their wellbeing.
Beverley Lock is our newly appointed SWIFt Wellbeing Coordinator (Supporting Wellbeing and Independence for Frailty). Beverley’s role will be to support people 50+ living with frailty with complex health issues who are often socially isolated, working together with people to develop a plan of action to help them to improve their health and well-being.
The SWIFt project has been running city wide for the past 2 years and Beverley has many years experience supporting families from diverse backgrounds on a 1-1 basis, to access groups and courses, dealing with issues such as housing concerns, money management, domestic abuse, isolation, child protection, behaviour management, health concerns, mental health and learning disabilities.
Due to the Coronavirus outbreak Beverley will be offering a telephone support service. If you are 50+, (or if you have a family member that meets the criteria) and living in the Chapeltown area and in need of 1-1 telephone support please contact Beverley on 07593436607 Monday-Wednesday 9-4.30pm and Thurs 9-12.30pm.
The SWIFt service can offer:
- Telephone support and assessments
- Signposting and information
- Home visits and assessments (Home visits are currently on hold during the Coronavirus Pandemic)
- One to one support to access services and community groups
