Working in partnership with Community Links and Age UK Leeds we are looking for an energetic and highly motivated individual to develop, promote and deliver our social prescribing service in North Leeds.
As a member of the team you will make a real difference to the lives of others. You will be a keen advocate of the benefits of supporting people with health and social care needs accessing primary care service to access support in their community by linking them to a network of groups and activities; enabling them to have improved health and wellbeing.
You will have relevant experience, be an effective communicator with a passion for excellence in your professional field. What’s essential is that you love what you do. We want you to enjoy coming to work every day and to care about your job, your colleagues and our service users.
You will be working with GP practices in North Leeds with access to the Connect Well office in the Reginald Centre, Chapeltown as well as Feel Good Factor’s offices also in Chapeltown
