Feel Good Factor is a health and wellbeing charitable organisation based in the heart of Chapeltown and has been in existence since 2003. We work in some of most deprived and diverse communities in Leeds providing activities and services to some of the most vulnerable and disadvantaged.

Partner organisations from health and care providers and the 3rd sector in Chapeltown, Harehills, Burmantofts and Richmond Hill are working together to deliver better wellbeing and health outcomes for people living with frailty, with a focus on developing and supporting independence.

Hosted by Feel Good Factor, the Care Co-ordinator role will be supported by Leeds Community Healthcare and local GP Practices and will play a key role in supporting people by co-ordinating their care and support in line with what is important to them and their individual needs.

What are we looking for?

As an enthusiastic Care Co-ordinator, you will be involved in proactive, person-centred care planning with local people. This is a role where you can make a real difference by supporting individuals to engage within their local community and pursue their personal goals, in line with their choices and preferences. The work will be undertaken in ways which are socially inclusive with the aim of empowering people to live healthier, happier and more independent lives.

People who have a background in working with local communities in improving health and wellbeing as well as those facilitating self-management or directly co-ordinating care or support would be suitable for this new role

You will be self-motivated with a desire to positively impact people’s lives and work collaboratively and effectively with colleagues and other organisations. Excellent interpersonal skills are key to this role, as are planning and organisational skills.

Experience and enthusiasm for monitoring and evaluation will be necessary as we are keen to learn from this innovative new role.
