Working in Partnership with Care & Repair Leeds and Leeds Black Elders Association we’re so excited about launching the next phase of our Healthy Lives Healthy Homes project very soon.

This project aims to improve health and wellbeing, provide positive activities for people with dementia and other long term conditions and provide information and support to their carers.

We want to reduce social isolation too, and we hope to recruit ‘Living Well Peer Mentors’ who will ensure there is ongoing support for our service users.

In practical terms we aim to increase safety and independence within the home, creating a safe and secure environment for everyone to enjoy a healthy home and healthy life.
